The .pdf document will need to be printed first then filled out by hand and faxed back, or scanned and emailed back to Smooth Hookers.
The Xcel document may be opend and filled out then saved and sent back by email.
Note: You must have either Microsoft Excel, or OpenOffice Calc (free download) to be able to open the document and fill out your order.
